Case Management
JOB SUMMARY:
The Case Manager will work under the supervision of the Clinical Director. The Case Manager will perform client intake, sign all intake documents, and perform group and individual sessions. The Case Manager will perform duties while adhering to the organization’s values and principals.
QUALIFICATIONS:
- A minimum of a High School diploma or GED equivalent.
- One year of experience in a mental health group home setting.
- Must have a completed TB test, CPR/First Aid certification, and health screening.
- Must have criminal background clearance.
PREFERRED QUALIFICATIONS:
- Master’s degree in social work or psychology.
ESSENTIAL DUTIES & RESPONSIBILITIES:
- Ensures all intakes documents are completed, signed, and dated for each client.
- Ensures client confidentiality and safety while maintaining appropriate boundaries with clients.
- Verifies that the Program Director has basic consumer information, medical history and all signed waivers and releases on new clients.
- Protects resident confidentiality, as required by State and Federal Privacy Laws.
- Completes incident reports.
- Review and complete admission checklist on each new client.
- Assist, teach and or supervise residents with program schedule activities and community rules.
- Observes client participation in a casual setting, such as group sessions, and at AA meetings to evaluate mental and physical status and progress.
- Confers with other case managers and the Program Director to provide input on client status and progress.
- Leads discussion groups and community meetings, as well as providing group instruction on substance abuse and related topics.
- Participates in the development, revision and implementation of individualized treatment plans and discharge/aftercare plans.
- Inspects clients’ belongings for contraband and other unauthorized items.
